It is internally threaded for comfort during insertion and has a countersunk top gem for a precision fit. The top gem is a 4mm bezel-set crystal cabochon gem which unscrews. The bottom gem is your choice of either a 4mm or 6mm fixed bezel-set crystal cabochon gem. The bottom gem is fixed in place for perfect orientation and does not unscrew. Both gems will be the same color. The bottom gem shown in the pictures is the 6mm.
